Key takeaways

  • The laboratory is reached during Act Two’s Moonrise Towers climax It is not normally accessible from the Shadow-Cursed Lands before the Mind Flayer Colony sequence.
  • Entering normally begins a large undead battle The encounter includes a Death Shepherd, Winged Horrors, Greater Zombies and Zombies, with additional undead raised during the fight.
  • The Brain Mapping Puzzle is mandatory for the best laboratory loot Solving it opens the Flesh-Wrought door and gives access to the Blade of Oppressed Souls and Waking Mind.
  • Waking Mind can grant a permanent defensive benefit Using the Mind-Archive Interface and choosing the appropriate dialogue can grant Githzerai Mind Barrier.
  • Mol’s Eyepatch is here Picking it up advances Find Mol and can provide an Urchin inspiration event.
  • The laboratory changes dramatically if Balthazar takes the Nightsong In that progression state, the party can awaken inside the Profane Wombs with the Profane Womb-Bound condition.

About this guide

Base game scope. This guide covers the Necrotic Laboratory proper in Baldur’s Gate 3. It does not treat the whole Mind Flayer Colony, the adjacent Morgue, the Tadpoling Centre, the Barracks or the Brine Pools as part of the laboratory, although those areas are included where they affect access, routes or nearby progression.

Overview

The Necrotic Laboratory is Balthazar’s working area inside the Mind Flayer Colony beneath Moonrise Towers. It is a compact but densely packed Act Two location containing an undead encounter, the Brain Mapping Puzzle, a locked illithid relief chamber, a Mind-Archive Interface, several brain jars, Dark Urge material and some of the colony’s most distinctive rewards.

The laboratory is not a free-roaming Shadow-Cursed Lands location. It is a named subarea of the Mind Flayer Colony, which the party enters during the Moonrise Towers sequence after the first confrontation with Ketheric Thorm. Its principal entrance is in the south-eastern section of the colony, and it connects directly with the separate Morgue area.

The laboratory is easy to mistake for a larger portion of the colony because its major set pieces lead naturally into other rooms. For clarity:

The Mind Flayer Colony

The Mind Flayer Colony is a temporary Act Two dungeon beneath Moonrise Towers. The party reaches it by jumping down into the tower after the fight with Ketheric Thorm on the Moonrise Towers Rooftop. The party must destroy the membrane blocking the initial passage before exploring the colony.

While inside the colony, the party cannot open the world map for fast travel or return to camp. This makes preparation particularly important. The Restoration Pod near the route to Ketheric’s lower arena can restore the party later in the colony, but it is not part of the Necrotic Laboratory.

The Morgue

The Morgue lies directly beside the Necrotic Laboratory and is a separate named subarea. It is run by Chop, a bugbear involved in producing Intellect Devourers. If Us survived the Nautiloid, Us can be found imprisoned there. The Morgue also contains the corpse of the Harper Olam, who carries the Necrotic Laboratory Spare Key. That key opens the locked Morgue Lab Door in the Morgue, not a hidden door inside the Necrotic Laboratory itself.

Areas that are nearby but not part of the laboratory

Do not fold the following areas into a Necrotic Laboratory directory:

These rooms belong to the larger colony or to nearby Moonrise locations. They have their own encounters, characters and quest outcomes.

Reaching the laboratory

Some guides describe the laboratory as being on the south-western side from a particular map orientation, but the more useful in-game reference is its position in the south-east of the colony map, near the entrance at approximately X: 715, Y: -50.

The normal route is

  1. 01
Enter the Mind Flayer Colony after the Moonrise Towers Rooftop confrontation with Ketheric Thorm.
  2. 02
Destroy the membrane in the opening passage.
  3. 03
Follow the colony’s route towards the south-eastern side.
  4. 04
    Enter the Necrotic Laboratory.
  5. 05
Prepare for an immediate undead battle.

Preparation checklist

The colony prevents camping and fast travel, so do not assume that you can leave after the opening fight to recover resources.

The central hexagram

The centre of the laboratory is dominated by a large necromantic hexagram. A Profane Womb sits at each apex. In the ordinary route through the area, this is primarily a visual and narrative landmark rather than a conventional puzzle or crafting station.

The hexagram has an additional progression-state function. If the party helped Balthazar reach the Nightsong in the Gauntlet of Shar and allowed him to take her away, the party later falls unconscious in the Shadowfell and awakens inside these containers. The resulting Profane Womb-Bound condition silences the characters and holds them in place. This is not a normal repeatable interaction available during standard laboratory exploration.

Balthazar’s study

A small study lies west of the central hexagram. Its desk contains research material and several notable items. The principal interactive object is the Mind-Archive Interface, fitted with a Slack-Skinned Head.

The interface allows the party to communicate with the brain jars found throughout the colony, as well as certain jars brought from earlier areas. It is one of the main reasons to search the laboratory carefully rather than treating the opening battle as the entire encounter.

The Rune Slates require Arcana checks to read. Their texts concern the fall of the Illithid Empire and the fate of the Elder Brain associated with the colony. The available text is reported even when the check outcome appears not to alter the written presentation, so players should treat the check as a possible dialogue or recognition gate rather than a guaranteed content lock.

South-eastern puzzle chamber

At the far south-eastern end of the laboratory is a neural button. Activating it projects the Brain Mapping Puzzle. Behind the projection is a Flesh-Wrought door.

The door cannot be opened by ordinary lockpicking or by Knock. The puzzle must be solved to gain access. Beyond it are the Desecrated Relief, the Blade of Oppressed Souls and the Waking Mind.

---

Entering the laboratory normally triggers a large undead encounter. The listed force consists of:

The Death Shepherd uses No Rest for the Wicked, raising additional reconstituted undead during the battle. The listed reinforcements include two Reconstituted Duellists, one Reconstituted Marksman and one Reconstituted Arcanist, with one of these appearing per round according to the laboratory’s encounter description.

Practical battle plan

The Death Shepherd is the encounter’s priority target. Allowing it to remain active gives the battle a continuing supply of reinforcements. Focus fire on it if your party can reach it safely, while using choke points, difficult terrain and control effects to delay the lesser undead.

The exact initiative order, critical hits and target selection are encounter-state dependent. The fixed element is the composition of the main undead group, not the precise sequence in which every reinforcement appears.

Useful general tactics include

  1. 01
Spread the party before combat begins if possible, reducing the risk of several characters being caught by the same area attack.
  2. 02
    Eliminate the Death Shepherd early.
  3. 03
Use radiant damage, area spells and high-ground attacks where available.
  4. 04
Prevent Winged Horrors from isolating weaker party members.
  5. 05
Keep a character near the centre of the room only if they can withstand being surrounded.
  6. 06
Search the room after combat. Several important rewards are easy to overlook near the skull pile and skeleton.

How it works

Each faculty must be connected to its matching copy on the opposite side of the projection. The selected paths must pass through intervening nodes without overlapping the other paths. The solution is a visual route puzzle rather than an Arcana or Sleight of Hand check.

Because the puzzle’s layout can be difficult to read on a small screen, rotate the camera and zoom in before selecting nodes. The important rule is that each coloured or labelled faculty must reach its identical destination while the four routes remain separate.

Rewards for solving it

The Brain Blast! inspiration event is background-dependent. The physical rewards are not restricted to Sage characters.

Suggested puzzle procedure

Do not waste time attempting to pick the Flesh-Wrought door or cast Knock on it. The laboratory description specifically identifies the puzzle as the required method of opening it.

---

The Desecrated Relief depicts the Grand Design. Interacting with it causes fragmentary descriptive phrases to be projected into the character’s mind by the Absolute. The Dream Guardian then comments on the image, explaining its relationship to the universal illithid goal of imposing the Grand Design and the ambitions of Elder Brains.

This is one of the laboratory’s clearest links between Balthazar’s necromantic work and the colony’s wider illithid history. The laboratory is not merely a room where undead are stored. It is also a place where the game connects:

The two Rune Slates in Balthazar’s study reinforce this historical context by describing the fall of the Illithid Empire and the fate of the Elder Brain associated with the colony.

---

Suggested puzzle procedure: steps

  1. 01
    Activate the neural button.
  2. 02
Identify all four starting faculties and their matching destinations.
  3. 03
Plan the longest or most constrained route first.
  4. 04
Trace each route through available nodes.
  5. 05
Check that no route overlaps another.
  6. 06
    Complete the projection.
  7. 07
Enter the chamber beyond the Flesh-Wrought door.
  8. 08
Search the skull heap and nearby skeleton before leaving.

How to use it

The Mind-Archive Interface is positioned near Balthazar’s study at approximately X: 699, Y: -117. It uses the Slack-Skinned Head to let a party member speak with the minds preserved in brain jars.

Two additional jars, Dark Mind and Slave Mind, originate in the Nautiloid prologue. Most of these provide dialogue and background information rather than a mechanical reward.

Waking Mind and Githzerai Mind Barrier

The Waking Mind is the important exception. It appears to belong to a Githzerai monk preserved by the illithids. Speaking with it through the interface and agreeing to the relevant exchange can grant Githzerai Mind Barrier, a permanent condition that gives advantage on Intelligence saving throws.

The exact dialogue options can vary with character choices and party composition, and some responses affect companion approval. The important practical point is to place the Waking Mind in the interface before leaving the laboratory and to complete its conversation rather than treating the jar as ordinary loot.

Named brain jars associated with the interface

The above is a directory of named jars associated with the interface, not a claim that every jar is physically inside the Necrotic Laboratory. Only the Waking Mind is located in the laboratory itself.

Main laboratory directory

Equipment and permanent rewards

The Blade of Oppressed Souls and Waking Mind are beyond the Brain Mapping Puzzle. The Braindrain Gloves and Circlet of Mental Anguish are on or near a skeleton beside the Waking Mind. The Resonance Stone is near Balthazar’s study and radiates a magical effect to nearby characters.

Equipment and permanent rewards

Mol’s Eyepatch

Mol’s Eyepatch is on the laboratory desk near the Mind-Archive Interface. Picking it up updates Find Mol. It can also grant the Amateur Optician inspiration event to a character with the Urchin background.

This is a particularly easy item to miss because it is placed among Balthazar’s papers and laboratory equipment rather than displayed as a quest object.

Resonance Stone

The Resonance Stone lies near the study. It provides the Steeped in Bliss effect to characters within approximately nine metres. This is a temporary area effect rather than a permanent party-wide upgrade, so decide whether you want to use it before moving on or entering another fight.

Items not belonging to the laboratory

The wider Mind Flayer Colony directory includes items such as the Infernal Rapier, Summon Us, Ketheric’s equipment and the Myrkulite Scourge. These should not be counted as Necrotic Laboratory loot:

The distinction matters because these items can be found in the same broad dungeon sequence but not in the laboratory proper.

---

Characters directly associated with the laboratory

Balthazar is not necessarily standing in the laboratory when the party explores it. His presence is chiefly established through the location’s ownership, research and narrative context.

Conditional characters connected to the wider colony

The following characters belong to the larger Mind Flayer Colony sequence rather than the laboratory’s permanent population:

Some appear only under particular story conditions, such as whether Us survived, whether Isobel was abducted, whether the Dark Urge is present, or how the party handled the Moonrise Towers and Gauntlet of Shar sequences.

---

Only Find Mol and the Dark Urge material have direct laboratory content. The other quests are listed because the laboratory sits within the same colony sequence, not because their principal objectives occur in this room.

The laboratory contributes to several quest threads

Balthazar and the Nightsong

The laboratory has an important progression-state variation tied to Balthazar’s fate and the Nightsong. If Balthazar is allowed to take the Nightsong from the Shadowfell, the party later awakens inside the Profane Wombs in the laboratory. The resulting condition prevents ordinary movement and silences the characters.

This is not an alternative route to extra loot. It is a consequence of a specific story path and should be treated as a major spoiler state.
